We are sincerely grateful to you for giving of your time and effort to complete this survey.
As colleagues in ministry, we too often keep to our work without the opportunity to share with others about what we are doing, and how we respond to challenges faced by our congregants. We are often too busy to take the time to hear about what others are doing, and how. This is especially true for specialized ministries, such as health ministry.
We known that following after Jesus’ example we are called to be healers, we often have little opportunity to tell the story of how our particular congregations carry out healing ministry. Thank you for your willingness to share the story of how health ministry is made flesh in your congregation.
We also promise you that the information you provide here will never be sold, bartered, loaned, given away or shared with any other entity, company, organization or individual. The information you have provided here will not be individually identifiable except by your own church officers, and only if they request it.
A report of survey results will be prepared by the National Council of Churches USA and made public in a written report.
